OMHA Permission to Skate Form
PLAYERS ATTENDING "AAA" OR JUNIOR LEVEL TRYOUTS:
For those interested in trying out for AAA or Junior Hockey, players are required to provide their desired team a "Permission to Skate" form prior to taking part in on-ice sessions.
To make your players intent known, please submit your request to the VP of Rep Hockey, so that the necessary paper work can be assembled and provided.
Form must be downloaded to your computer, updated with the player information, signed by the parent/guardian and emailed directly to SSMHA's Director of Rep Hockey [email protected]
