Concussion Forms
As many of you might be aware, in June 2018, the Ontario Government passed legislation to increase awareness about concussions as a result of 17 year old Rowan Stringer, who died from second impact syndrome after experiencing 3 concussions in six days.
As part of this legislative requirement, amateur athletes, coaches, officials and parents are required to sign off on documentation provided by the governing association about the risks of concussions, what concussions are, and how to recognize the signs and symptoms. SSMHA developed a form and requires ALL players, parents, bench staff and officials to sign off before the player steps out on the ice.
Thank you to everyone who has signed off, but for those who have NOT signed this form, we do require this before your next ice time. Parents are required to sign off on all players under the age of 18. If you do not have a form, please contact your coach to access one. Have a great season and play safe!
